Judy I do not have an ISBN number as this is online with ALEKS at college and do not know what textbook they use. Sorry.

Line Form: y = mx + b
Find m and find b
------------------------
m = 5
Find b:
4 = 5*2 + b
b = -6

Equation: y = 5x - 6
