Put it in slope-intercept form, y = mx + b
y-y1 = m*(x-x1) (x1,y1) is the given point, (3,4)
y-4 = 6*(x-3)
y-4 = 6x-18
y = 6x-14 (slope-intercept form)
6x-y = 14 (standard form)
