I am unsure how to put that into standard form or determining the slope-intercept form

The slope is 5/6
Use slope m = diffy/diffx = 5/6

It's the slope of the line thru the points, not the slope of the coordinates, tho.

Use the slope and either point to find an equation:
y - y1 = m*(x - x1) where (x1,y1) is one of the points.
y - 3 = (5/6)*(x - 2)
Multiply by 6 to eliminate the fraction.
6y - 18 = 5x - 10
---
5x - 6y = -8 (Standard form)
y = (5/6)x + 4/3 (slope-intercept form)
