Using the average rate of change equation, I got the slope of 22 but am unsure of how to find the rest of the equation.

Slope is unimportant to the description and question.
Standard Form for the equation of a parabola follows ;
The vertex is (h,k).
The description gives (h,k), so these are easily substituted into the basic equation form. The other given point allows solving for the factor, "a". You can do this symbolically and then substitute according to the two given points to evaluate "a". Using "a", h, and k, just fill-in the standard equation for the parabola.
