Look at the lesson linked through my profile, "Completing the Square to Solve General Quadratic Equation". You will want to change goal a little bit and put the equation into standard form, from which you can read the vertex point coordinates. The x-intercepts are the general solutions to a quadratic formula. For y-intercept, just let x=0, which you can do with the general form equation which you are given.
