Slope: 5; y-intercept: (0,-2)
The slope intercept form of a line is: , where m=slope and (0,b)=y-intercept.
slope=m=5 and y-intercept=(0,b)=(0,-2)
y=5x+(-2)

To graph this, plot the y-intercept (0,-2) at -2 on the y-axis (The vertical one.)
From that point, use the slope,m=rise/run=5/1. Go up 5 units and right one unit and plot another point at what should be, (1,3).
Connect the two points and you should have a line that looks like:
