First off, find the slope of your first line.
Re-arrange it into slope intercept form,
Then find the slope of the second line, using the slope relationship of perp. lines,
Then use the point slope form,
Then re-arrange so that you have no denominators and x and y are on the left hand side, constants on the right hand side.
