Since the exam counts 20%, the course grade counts 100%-20% or 80%
The formula is

Final grade = 80% of course grade + 20% of exam grade

Final grade = 80% of 86% + 20% of 0%

Final grade = (.80)(.86) + (.20)(0)

Final grade = .688 = 68.8%
