How do you rewrite an equation 2y+5x=-8 in slope intercept form, which is the slope and which is the y intercept

The slope/intercept form: y = mx + b, where m is the slope; b is the y intercept
2y + 5x = -8
2y = -5x - 8
Divide equation by 2
y = x - 4
or
